I offer service to patients of age 6 months or above

I offer  to provide care to patients of age from 6 months and above.

I offer to practices but I limit or refer IDDM patients, decline joint injections, or any pain medication patients and prefer refer them to other experts. I do not refill narcotics

I try and limit PEDS patients under 6 months. In my past I have had  experiences 7 days  old infants. But, I believe many of that  age need a septic  workup and admitted.

I refer all Pain patients to pain management facilities.

I wish to offer service to hospital owned but due to the difficulty dealing with the JACHO  credentialing I am  not able to go thru the process. 

At present I have not renewed my DEA license. I refer narcotic prescriptions to the supervising MD. This may change in the future.

I attempt to provider service to practice. I am willing to offer all my diplomas and PA license upon request, criminal background checks, drug tested, data bases. etc. (at your expense). I do not share other MD’s or Providers to be called for past references. It is too difficult and a hardship for past MD’s. I respect their time and practice and time to care for their patients.

At  present, I ask the practice to provide and cover malpractice Insurance for my service. My experience most insurance companies  understand and provide coverage for locums Provider and umbrella my service under your existing policy. In the future I may obtain my own policy. But,not at present.

I request due to the time , effort, and distance  a minimum hours per week and length of the contract. This depends on the distance from home to practice. 

I offer service and am willing to provider service more then 40 hrs per week at regular hourly wages without overtime.

I expect an agreement for the length of service to your practice. I go from contract to contract. I am willing to extend to help you if your needs have not been made for a perm Provider. But, due tom the distance I expect a min notice of 2 weeks ( 72-80 hours) for termination. This allows me to seek other future contracts.

I offer  two different options to practices for pay.

OPTION 1: My rate is $65/hr and request practices to provide for lodging. It is cheaper to obtain lodging places that offer weekly or monthly rates ( the longer the stay the cheaper per day decreases as opposed to daily lodging )

OPTION 2: I base my service at $65/hr. I do not charge for overtime.  Your practice does not offer me any benefits.Therefore my hourly rates are higher so I can provide my benefits for myself i.e. retirement, health insurance. vacations, licenses and CME’s. Some practices have a difficult offering lodging for a locums Provider. Therefore, I offer my hourly rate will increase if I attain my own lodging. My goal is to obtain the most reasonable lodging in your area. I find  the weekly rates and divide the rates and add per hour for the lodging + $65/hr. Many practices want help but decline to pay for the lodging. So my future rates to your practice may be higher based on the expense of lodging in your area.

You are more then willing to contact other locum companies but,you will never find a cheaper rate then a what I offer nor more experienced Provider

The option is choice of the practice.
